Home Health & Hospice
Home Health provides medically necessary nursing care following discharge from either hospital to home or from a Skilled Nursing to home. Doctors typically refer a patient that is ready to be discharged to a Home Health Agency. Home Health services are covered by Medicare in much the same way that prescription medications are covered; the patient’s primary doctor will prescribe what is required and the “prescription” requires renewing for services to continue.
Hospice services provide palliative care and medically necessary nursing services covered by Medicare for the elderly and by MediCal for the disabled. These organizations deliver in-home visitation care for end-of-life support. Hospice services also include chaplain and counseling assistance.
